Simply complete the program application and send it to us by mail or fax along with your bylaws and your most recent financial statement. We will provide you with a no-obligation quotation within 3 business days. The application is in .pdf format and requires Adobe Acrobat Reader to be opened. The program policy is subject to a minimum annual premium of $850 and an FRP Canada $25 administration fee. Coverage is available upon request and payment must be received in full within 30 days of the effective date of your policy.
